Position: Primary Care Nurse (LPN or RN)

Employment type: Permanent, Part-Time, 0.8 FTE (64 weekly hours).

Location: The Primary Care Team Nurses are decentralized positions that work in busy medical clinics with our member-Physicians, mostly concentrated around west Edmonton. This specific posting is for Terra Losa Medical Clinic located at 9760 170 St NW.

Objective: Being a nurse on our Primary Care Team means that you get to utilize your full scope of practice alongside family physicians and other healthcare professionals in an interdisciplinary environment.

ABOUT THE EDMONTON WEST PRIMARY CARE NETWORK:

With a mission to support family physicians in improving primary care through relationships, collaboration and innovation, Primary Care Networks (PCN) are groups of family doctors who work with other health professionals to coordinate the comprehensive delivery of primary care services for their patients. Our team of health professionals include Nurses, Dieticians, Pharmacists, Behavioral Health Consultants, Social Workers and Exercise Specialists who collaborate to provide comprehensive primary care services for the community.

Previous Experience:

• Three to five years of professional nursing experience
• Previous experience in primary care setting is a strong asset
• Chronic disease management
• Case management and/or health system navigation
• Proficiency in Microsoft Office Suite (including Word, Excel, and Outlook)
• Demonstrated experience using EMR and Netcare

License/ Certification(s):

• Current CLPNA or CRNA Registration and BLS CPR is required
• Valid driver’s license and reliable access to a vehicle
• Certification as a Certified Diabetes Educator (CDE) is an asset
